Drake Medox is currently seeking experienced Case Managers to join our dedicated team based in the beautiful Bondi Junction.

We're Looking For:

  • Experienced Case Managers with strong knowledge of the community or aged care sector

  • Prior experience in coordinating home care services (HCP or private) preferred

  • Excellent interpersonal and communication skills

  • Highly organised, detail-oriented, and proactive problem solver

  • Compassionate and client-focused with a genuine passion for care

  • A current driver's licence and willingness to travel locally for assessments

Your Key Responsibilities Will Encompass:

  • Conducting in-home or facility-based client assessments

  • Developing, implementing, and reviewing personalised care plans

  • Coordinating service delivery and delegating tasks to care staff

  • Managing client budgets (especially for government-funded programs)

  • Liaising with families, health professionals, and service providers

  • Acting as a trusted point of contact and advocate for client wellbeing

About You:

To be successful in this role, you will ideally have:

  • Qualifications in care, community services, or allied health (preferred but not mandatory)

  • Previous experience in case management or community-based care

  • A proactive and client-focused approach

  • Strong attention to detail and time management skills

  • Police Check, Working with Children Check, full working rights in Australia

  • Access to a reliable vehicle
