Senior Complex Case Manager

The Senior Complex Case Manager is responsible for supporting the provision of high quality, veteran centric, trauma-informed case management practice across the Coordinated Client Support Branch.

Providing clinically informed case management to veterans presenting with challenging behaviour and/or with complex needs.



Duties:

  • Provide case management and consultation support to DVA
  • Provide an escalation point for case managers
  • Contribute to the preparation of high-level client summaries and briefs, including maintaining high quality documentation
  • Undertake a range of client assessments including intake assessments and risk assessments.
  • Provide support to staff with a speciality in challenging behaviour
  • Be available for ad hoc consultation across the branch
  • Contribute to the development, use and review of clinically informed policies, procedures, and practice tools, including the development of training and education processes



Essentials:

  • Tertiary qualifications (bachelor's degree or higher) in a relevant health field including nursing, psychology, occupational therapy, or social work
  • At least 5 years' experience working as a senior case manager or equivalent role
  • Strong experience working with challenging behaviour
  • Extensive experience working within a mental health or community case management setting, including working with people who present with complex mental health conditions
  • Experience in coordinating and assisting in the management of a caseload of complex and potentially at-risk clients in community mental health teams
  • Excellent written and oral communication skills
  • Proficiency with Microsoft Office
  • Experience and interest in triage, assessment, and case management via telephone
  • Demonstrated commitment to ongoing professional development

Highly Desirable:

  • Registration with AHPRA or eligibility for registration with AASW
  • Qualifications in Forensic Psychology

Desirable:

  • Experience in working with military or veteran mental health services
  • Experience working in a forensic setting
  • Expertise in supporting clients experiencing the impacts of substance abuse, mood, and anxiety disorders (including PTSD)
