Aged Care Placement Consultant

About the Role:

As an Aged Care Placement Consultant, you'll work closely with individuals and their families to identify suitable residential aged care options tailored to their care needs, financial situation, and lifestyle preferences. This is a dynamic role that combines client-facing support, relationship management, and administrative coordination. You'll carry your own caseload while being part of a collaborative and caring team.

What You'll Be Doing:

  • Provide expert advice and compassionate support to clients navigating the aged care system

  • Identify and present suitable aged care options based on individual needs and preferences

  • Liaise with aged care providers to coordinate facility tours, admissions, and placements

  • Communicate with hospitals, discharge planners, and external stakeholders to facilitate timely discharges

  • Support clients with Centrelink and aged care admission paperwork

  • Maintain accurate client records and participate in case reviews and team meetings

About You:

We're looking for someone who brings compassion, energy, and a client-first approach. Ideal candidates will have:

  • Experience in the aged care or health sector (residential admissions highly regarded)

  • Knowledge of aged care fees (RADs, DAPs, means-tested care fees) - or a willingness to learn

  • Exceptional communication and interpersonal skills

  • Strong organisational skills and the ability to manage multiple priorities

  • Proficiency with IT systems and CRM software

  • A genuine desire to help people through times of uncertainty and change
