ABOUT THE ROLE

Reporting to the Branch Manager, the Clinical Coordinator is responsible for providing support to our staff, develop and review care plans, as well as manage our client relationships to ensure they are receiving the best possible care.

Drake Medox is currently seeking a passionate and dynamic Registered Nurse, who can provide Clinical Coordination for our complex care clients in the community within the disability sector.

Tasks and responsibilities, but are not limited to:

  • A mix of office based and client facing interactions.
  • Work remotely and autonomously when required.
  • Consulting and implementing client care plans and clinical documentation.
  • Client program reviews and regular Client visits including creation of Client's Care Plans and updates.
  • Liaising with external stakeholders i.e., Allied Health Team, Support Coordinator etc.
  • Running Inductions, Education and Training workshops for our support staff. 
  • Conducting competency assessments for our staff.

To succeed in this position, you will have the following:

  • AHPRA Registered
  • Passionate about making a difference for those living with a disability in the community.
  • Understanding of the NDIA/NIISQ and other funding bodies an asset (however not essential)
  • Consider yourself resilient, driven and an effective communicator.
  • A team player that is willing to perform all-rounder support when needed.
  • Strong computer skills.
