Job Description

Drake Medox is currently seeking a passionate and dynamic Registered Nurse, who can provide Clinical Coordination for our complex care clients in the community within the disability sector.

About the role:

We are looking for a Clinical Coordinator to provide advice, planning and review of all aspects of clinical care for our clients. Some of the key responsibilities of this role are:

  • A mix of office based and client facing interactions.
  • Work remotely and autonomously when required.
  • Consulting and implementing client care plans and clinical documentation.
  • Client program reviews and Regular Client Visits.
  • Liaising with external stakeholders i.e., Allied Health Team, Support Coordinator Etc
  • Running Inductions, Education and Training workshops for our support staff. 
  • Conducting competency assessments.
  • Business development opportunities to help grow our NSW footprint.
  • Recruitment activities – predominately health professionals in the disability field.

About you:

  • AHPRA Registered
  • Passionate about making a difference for those living with a disability in the community.
  • Understanding of the NDIA/NDIS and other funding bodies an asset (however not essential)
  • Consider yourself resilient, driven and an effective communicator.
  • A team player that is willing to perform all-rounder support when needed.
  • Strong computer skills.
